Yorkshire Street Pharmacy
Registering for our NHS Prescription Service couldn’t be easier. Submit this form and prescriptions issued by your GP will be sent electronically to your nominated Pharmacy+Health branch.
Our pharmacists use the very latest technology to safely dispense your medicines and are available to provide you with advice and other healthcare services. Please provide your mobile number and email so we can inform you when your prescription is ready. Depending on your postcode, you may be able to choose home delivery.
Once you have registered, you can continue to request your repeat medicines from your GP. Alternatively, you can join the 30 million people who use the NHS App and request your next repeat prescription that way.
